Repeat Prescriptions

health_tech1If you are on long-term treatment and need regular prescriptions you will be given a repeat prescription slip. The counterfoil is from a computer printout as our repeat prescriptions are now computerised. It allows you to get repeats of your medication either by requesting online or bringing it to the reception desk. Please tick the appropriate box for the medication you require. If you do not tick which medication is required you will not be prescribed any of those listed.

Please give at least two working days notice before collection of your prescription. This information is contained on the 2nd page of your computerised prescription slip.

To avoid any errors we do not take repeat prescriptions over the telephone.
